A commitment to providing quality care to individuals appropriate for home care is shared by all members of the NHHC team. Our team will work closely with the physician, discharge planner, family, and other potential referral sources to assure appropriate care is provided. When necessary, a nurse/therapist will visit with the patient and family to assess his or her needs. Under the direction of the patient's physician, with input from the patient and nurse/therapist, an individualized Plan of Care, or plan of treatment, is developed. |
